Membership - JOIN or RENEW
Provider Membership is available to organizations/agencies/companies doing business in the state of Ohio who provide home health, homemaker, hospice, case management, therapy services that can attribute 50% or more of their business to home care, or other home health care services exclusive of home medical equipment suppliers. Each provider member is eligible to cast one vote. Each Provider member should fall under one of the following Auspices: Homemaker/Home Health Aide, Hospice, Institution Based, Official, Private Non-Profit, Proprietary, Voluntary Non-Profit, and Private Duty.
Annual dues for all provider/voting members are based on gross revenue for the 2014 fiscal year generated in Ohio from all locations and all patient revenue payers. A provider/voting member is a business or corporation/entity doing business in Ohio under common ownership or control, or board direction, including all additional locations, sub-units, and programs providing in-home patient services and hospice.

The Startup/Conditional Provider member category is designed for startup home care and hospice agencies that otherwise do not qualify for Provider Membership. Upon obtaining Medicare/Medicaid certification for home health or licensure for hospice during the first member year, the Startup/Conditional Provider member agency will remain a member in this category for the remainder of the current membership year. The agency will then apply for Provider Membership for the upcoming membership year. This is a non-voting membership.
Hospice - To qualify as a Startup/Conditional Provider a hospice agency must be in the beginning process of obtaining hospice licensure in Ohio and does not have an active Ohio license.
Home Care - To qualify as a Startup/Conditional Provider a home care agency must be in the process of obtaining their Medicare/Medicaid certified number and does not currently have an active Medicare/ Medicaid certification number for any Ohio agency location. The home care agency must also be listed on the Ohio Department of Health website with a status of “Pending”.

An Ohio institution of higher education may join OCHCH at no cost and then through its membership may sign-up students for complimentary membership in OCHCH. The individual student must be listed as a full-time student, in good standing, on the department enrollment list provided by the Education member institution to OCHCH.
Any individual who otherwise qualifies under any other membership category (Provider, Affiliate, Startup/Conditional, Affiliate - Individual, Student, or Retired) must join through the appropriate qualifying member category. The Education membership is not applicable for an individual who is in a paid position within the home care and/or hospice field. This is a non-voting membership.

The Retired member category is for retired individuals that are no longer working in the home care and/or hospice industry. In order to qualify for this membership, the individual must have previously worked for an OCHCH Provider member within the last five (5) years.
Any individual who otherwise qualifies under any other membership category (Provider, Affiliate, Startup/Conditional, Affiliate - Individual, Education, or Student) must join through the appropriate qualifying member category. The Retired membership is not applicable for an individual who is currently working in the home care and/or hospice field. This is a non-voting membership.
Total Dues: $50
Affiliate Membership is open to home medical equipment suppliers; organizations providing products and/or services to home care providers; other organizations doing business with home care providers or interested in the mission of OCHCH, such as: consultants, insurers, intermediaries, managed care organizations, and other trade and associations. Organizations eligible for provider membership may not join the OCHCH at the Affiliate level. This membership is not applicable for anyone employed by a company providing hospice or home care.

Affiliate - Individual
The Affiliate - Individual member category is designed for individuals that are sole-proprietors, or single employees of their company and is providing services that support the home care and/or hospice industries.
Any individual who otherwise qualifies under any other membership category (Provider, Affiliate, Startup/Conditional, Education, Student, or Retired) must join through the appropriate qualifying member category. The Affiliate - Individual membership is not applicable for an individual who is currently working in the home care and/or hospice field. This is a non-voting membership.
