Award of Excellence Nomination

The Award of Excellence recognizes an outstanding licensed professional in home care and hospice. Nominees must have at least three years of experience in providing patient care (past or present position) and must be employed by an OCHCH member. Nominations may include personnel currently in administrative or supervisory positions. This award recognizes someone working in the home care and hospice industry.
